P2195 and P2197: oxygen sensor signal biased or stuck lean
The sensor is not out of range and not dead. It is simply not moving the way a working sensor moves, and the ECU has noticed the difference.
A healthy upstream oxygen sensor never sits still. It swings rapidly between rich and lean indications as the ECU deliberately oscillates the mixture around stoichiometric, and the speed of that swing is itself a measure of sensor health.

What “biased” means
Not stuck at an extreme, and not out of range. Biased means the signal sits consistently to one side of where it should sit, or swings around the wrong midpoint.
The ECU detects it by comparing the sensor’s behaviour with what the fuel trims say. If trims are strongly negative — the ECU removing fuel — while the sensor still reports lean, something does not add up.
The critical question: is the sensor lying, or telling the truth?
This decides everything.
If the sensor is wrong, the mixture is fine and the ECU is being misled into adding fuel it does not need. The engine then runs genuinely rich because of a sensor fault.
If the sensor is right, the mixture really is lean, and the sensor is doing its job.
Fuel trims separate the two:

A sensor reporting lean while the ECU is removing fuel is a contradiction, and the sensor is the thing that must be wrong.

Causes of a biased sensor
Contamination
The leading cause. The sensing element is poisoned by:
- Coolant, from a head gasket leak. If you replace sensors repeatedly, find out why.
- Oil, from worn valve stem seals or heavy blow-by.
- Silicone, from the wrong sealant used during engine work. Self-inflicted and avoidable.
- Excessive fuel additives, particularly metallic ones.
Age
Response speed degrades gradually. A sensor near 150,000 km can be lazy without setting a code, and biased shortly afterwards: P0133 oxygen sensor slow response.
An exhaust leak upstream
Air drawn in through a leak before the sensor makes it read lean — accurately, for the gas it is sampling. The sensor is fine and the reading is wrong.
This is the most commonly missed cause. Check the manifold, the flexible section and the sensor bosses before condemning anything.
Heater problems
A sensor that never reaches full operating temperature responds slowly and can sit biased: P0030 to P0037 heater circuit and P0053 and P0054 heater resistance.
What the waveform should look like
Log the upstream sensor at a warm, steady idle for thirty seconds.

A signal parked near 0.45 V is the ECU’s bias voltage on an open circuit, not a perfect mixture. It looks plausible, which is exactly why it fools people.
Consequences
- The mixture control loop chases a false reference, so the engine runs rich or lean constantly.

Sequence
- Record fuel trims alongside the sensor reading — the contradiction is the diagnosis.
- Log the waveform at warm idle.
- Check for exhaust leaks upstream of the sensor.
- Check heater codes and cold-start response time.
- Look for contamination sources if this is a repeat failure.
- Replace with the correct part, not a universal with spliced wiring.
- Clear trims afterwards and confirm they settle near zero.
